Patient Consent & Appointment Policy
Please read this policy before booking an appointment. It explains what we ask of every patient and what informed consent means at Quantum Physiotherapy & Sports Rehab.
1. Purpose of This Policy
This policy sets out the responsibilities you agree to as a patient when booking and attending an appointment with us, and explains how informed consent works before any physiotherapy assessment or treatment begins.
2. Patient Responsibilities
By booking an appointment, you agree to:
- Provide accurate and complete personal and contact information at the time of booking.
- Inform our therapists of any relevant medical history, current medications, injuries, surgeries or health conditions before your assessment begins.
- Arrive on time for your scheduled appointment, allowing our therapists to give you their full session.
- Bring along any previous medical reports, scans, X-rays or referral letters where applicable, as these help our therapists plan safe and effective treatment.
3. Understanding Your Consultation
Booking and paying for a consultation through our website reserves your appointment slot and covers the initial assessment only. It does not automatically include every physiotherapy treatment, rehabilitation package, home visit or specialised procedure — these are recommended and charged separately only where clinically appropriate, and only after your therapist has assessed you in person.
4. Treatment Plans May Change After Assessment
5. Informed Consent
Before any assessment or treatment technique is carried out, your therapist will explain what it involves, why it is being recommended, and any reasonable risks or alternatives, in language you can understand. You are free to ask questions at any time, and you may decline any part of a proposed treatment without affecting your right to receive other appropriate care.
6. Withdrawing Consent
You may withdraw your consent to a particular treatment technique at any point during a session, including while it is in progress, simply by informing your therapist. Your comfort and wellbeing always come first.
7. Confidentiality
Any medical information you share with us is treated as confidential and is used only to plan and deliver your care, in line with our Privacy Policy.
8. Related Policies
This policy works alongside our Terms & Conditions, Medical Disclaimer and Cancellation & Refund Policy. If you have questions before booking, please visit our Contact Us page.
